If your child has medication left in the clinic, please pick it up prior to the summer break. Medication will be disposed of during June if prior arrangements are not made with clinic personnel.

As a reminder, new medication forms are required every school year. A physician’s written order is required for any prescription medication at school, whether it is administered in the clinic or self-administered by the student. Middle and high school students are permitted to carry and self-administer non-prescription medication with a signed parent consent form. A physician’s form is required for non-prescription medication for elementary students. Forms are available on the website at this link.

If your child requires diabetic care, an inhaler, and/or emergency treatment for an allergy, you may also access the forms online at the same location. School personnel are not permitted to give expired medication; therefore, be aware of the expiration dates of all medication.
Please contact the school office or clinic staff if you have any questions.

If you would like a DVD of the graduation ceremony, please send in your order (Graduation DVD Order Form) to the main office by Thursday, May 24.  The cost is $15, and checks may be made payable to OHS.

The Baccalaureate service for the class of 2012 will be held Thursday, May 31 at 7PM at Powell United Methodist Church. All graduates and their families are invited to attend this service which recognizes and celebrates commencement.  Members of the graduating class will be conducting the service.  Jared Mills will be the keynote speaker. Please contact Linda Shank at linda_shank@olentangy.k12.oh.us with any questions.
